*Regarding the High-Cost Medical Expense Benefit System: Patients who provide their MyNumber Card do not need to provide a "Limit Eligibility Certificate."
*If you do not wish to use your MyNumber Card as a health insurance card, please bring your standard Health Insurance Card. If you have a MyNumber Card but have not yet registered it for use as a health insurance card, you can complete the registration process using the card reader at our clinic. (You can also complete the process yourself via the MynaPortal).
